Dear Parent / Guardian:

We are off to an exciting year and would like to go over some concerns related to the school health office.

Medication In School - New Jersey law requires a written and signed order from a legal prescriber for medications being given at school as well as signed consent from a parent/guardian. Tylenol and Ibuprofen can be given out with parent permission. An Asthma Action Plan, completed and signed by your child’s physician, is required to be on file for those students diagnosed with asthma.

Screenings – Each student will be screened for height, weight and blood pressure. Students in 7th grade will receive a hearing screening. We will work with the Gloucester City Lions Club will conduct vision screenings for students in 4th, 6th and 8th grade.  You will be notified of any need for follow-up with your personal physician.

Immunizations – Please remember that 6th-grade students are required to receive a tdap vaccine and meningococcal vaccine. If your child has already received these immunizations, please send verification to our office as soon as possible. If we do not have a current immunization record for your child, a letter was emailed to you last week.

Sports Physicals -  Sports Physicals have to be completed on the state reporting form. Please keep in mind that once you submit a completed packet, it is sent to the school district physician for approval. It often takes a few days before your child can begin to participate in their desired sport.

Contact Information - Please be sure to update your contact information as needed throughout the school year so that we are able to reach you. If an illness or emergency occurs during the school day we will call you or your emergency contacts as soon as possible.

Healthy Habits - Prevention is the best way to maintain a healthy school environment. Frequent hand washing, maintaining a healthy diet, daily exercise and getting plenty of rest are the best ways to prevent illness.

Please refer to the “Health Services” tab on the district web page for further information or any forms you may need. If you have any questions, needs or concerns feel free to contact us at any time.
